Take Care provides care in any home setting—whether it be in one’s own home, an assisted living or nursing facility, or in the hospital. Private duty home health care is one-on-one care, at the level you (or your physician) may request:
- Registered Nurse (RN)
- Licensed Practical Nurse (LPN)
- Certified Nursing Assistant (CNA)
- Home Health Aide (HHA)
- Companions
Home health care may be scheduled for as little as one hour a day to as much as 24 hours a day, 7 days a week. Our New Services team communicates with you to determine the frequency and duration of this care.
Every Take Care caregiver—regardless of his or her level—performs light housekeeping, such as, preparation of meals, keeping living areas tidy, bed making, and laundry.
A courtesy initial visit will be performed by a New Services Registered Nurse so we may personally meet with you to help determine your personal care needs.
